Eleven years after the occasions in Burma, Vietnam War veteran John Rambo lives in Bowie, Arizona at his perished father's pony farm, which he makes do with his old companion, Maria Beltran, and her granddaughter, Gabrielle. Gabrielle reveals to Rambo that a companion of hers, Gizelle, has discovered Gabriela's natural dad, Manuel, in Mexico. Both Rambo and Maria advise her not to go to Mexico, however, Gabrielle covertly drives there to inquire as to why Manuel surrendered her and her mom years prior. Gizelle drives Gabrielle to Manuel's condo, where he icily reveals to her that he never truly thought about Gabrielle or her mom.
Gizelle takes a crushed Gabrielle to a close-by the club, where Gabrielle is sedated and hijacked by the authorities of a Mexican cartel. In the meantime, Maria advises Rambo of Gabrielle's vanishing in Mexico. Rambo races to Mexico and questions both Miguel and Gizelle about Gabrielle's whereabouts. Gizelle hesitantly drives Rambo to the club where Gabrielle was most recently seen and defies El Flaco, the man who last talked with Gabrielle. A strange lady, Carmen Delgado, tails Rambo as El Flaco drives him to Gabrielle's area. Rambo is quickly faced, beaten, and set apart by the cartel, drove by the siblings Hugo and Victor Martinez. They take his driver's permit with the area of Rambo's farm and a photograph of Gabrielle, whom Victor perceives. The cartel promises to abuse Gabrielle further because of Rambo's activities.
